Mather Hospital’s innovative Rapid Access Center is a point of entry for care for individuals age 18 and up who are experiencing a mental health or substance use disorder issue and do not know where to go for help. A clinical social worker and nurse practitioner performs an initial assessment and connects individuals seeking help with the appropriate services. The center is located at 100 Highlands, Port Jefferson (across the street from the hospital). Understanding the right type of Mental Health care that you need can be stressful.  The Rapid Access Center can help to navigate and connect you to the mental health and substance use services that best meet your needs. We also provide brief time-limited interventions that will bridge you to the appropriate level of care.

Currently we see patients 18 years or older.

Monday through Friday 9am to 5pm. No weekends, no holidays

The RAC length of stay is between 1 and 4 sessions.  The goal is to provide you with assessment and referral to the appropriate level of care.

Our staff includes Social Workers and Nurse Practitioners.

If during your assessment medication management is recommended for you, and we cannot connect you with a provider in a timely manner, we will make an appointment with our Nurse Practitioner.  Medication Management can include but is not limited to Psychiatric Medication Management, Medication Assisted Treatment for Addictions and Long Acting Injectables.

Every person that comes to the RAC will have an after-hours crisis/safety plan to use for the duration of your brief treatment with us and until you connect with your provider.

Insurance verification is done prior to coming to the RAC.  Insurance is a contract between you and your insurance carrier.  Any special benefit criteria, co-pays or deductibles are the responsibility of the patient.

Finding the right help and support you need for your mental health needs can be overwhelming. The RAC is designed to assist patients in accessing comprehensive mental health and substance use care through assessment, brief treatment and referral to treatment.

No, we are not able to provide medical clearance or CPS evaluations for patients.  We can refer you to a provider who can assist you further.

No, we do not prescribe new or refill narcotic prescriptions at the Rapid Access Center.
