Mather’s Transitional Care Unit (TCU) provides focused care to patients who are not ready to be discharged home or to another facility. TCU patients include recent stroke sufferers, post-surgical patients, those receiving therapy for systemic infections and those who need physical rehabilitation. The TCU was awarded a 5-star rating by medicare.gov, which is based on 3 sources: health inspections, staffing, and quality of resident care measures. In 2017, the TCU ranked as a Top Performing Nursing Home in U.S. News & World Report’s Best Nursing Homes. In 2016, the TCU was awarded the Healthstream Excellence through Insight Award for overall patient experience.
Mather’s TCU is a beautifully appointed unit with hardwood flooring and an openness that makes patients feel at home. It is a quiet, healing environment that encourages mobility and independence. The unit has four private and six semi-private rooms, a bright and cheerful therapy room and a dining room/common recreational space where patients can visit with family and friends.
Mather’s TCU is staffed by a highly skilled, experienced team of healthcare providers including pharmacists, physicians, registered nurses, certified nursing assistants, physical therapists, occupational therapists, a speech therapist, registered dietician, licensed medical social worker and a recreational therapist. TCU patients have access to all the medical staff specialties, as well as the pharmacy, clinical nutritional services, respiratory therapy, diagnostic imaging and the diagnostic laboratory.
