Mather’s Transitional Care Unit (TCU) provides focused care to patients who are not ready to be discharged home or to another facility. TCU patients include recent stroke sufferers, post-surgical patients, those receiving therapy for systemic infections and those who need physical rehabilitation. The TCU was awarded a five-star rating by medicare.gov, which is based on three sources: health inspections, staffing, and quality of resident care measures. The TCU was given a short-term rehabilitation rating of High Performing by U.S. News & World Report’s 2022-2023 Best Nursing Homes.
Mather’s TCU is a beautifully appointed unit with hardwood flooring and an openness that makes patients feel at home. It is a quiet, healing environment that encourages mobility and independence. The 16-bed unit has four private and six semi-private rooms, a bright and cheerful therapy room, and a dining room/common recreational space where patients can visit with family and friends.
The TCU is staffed by a highly skilled, experienced team of healthcare providers including pharmacists, physicians, registered nurses, certified nursing assistants, physical therapists, occupational therapists, a speech therapist, registered dietician, licensed medical social worker and a recreational therapist. TCU patients have access to all the medical staff specialties, as well as the pharmacy, clinical nutritional services, respiratory therapy, diagnostic imaging, and the diagnostic laboratory.
