
Transitional Care Unit
In 2007, Mather Hospital launched the first Transitional Care Unit (TCU) on Long Island and one of only five in New York State under a five-year state pilot program. Due to the program’s success, the TCU has become a permanent service at Mather.
Our TCU ensures that patients receive the focused care they require before being discharged home or to another facility for those requiring extended therapies. Fostering independence is the key to the TCU’s mission.
Patients considered for the TCU include:
- patients needing physical rehabilitation
- recent stroke sufferers
- post surgical patients
- patients undergoing therapy for systemic infections
- patients newly diagnosed with diabetes and requiring patient/caregiver education
- new ostomy patients
- patients requiring IV therapy with frequent laboratory tests; IV anticoagulation therapy
- patients requiring high flow oxygen and close respiratory therapy monitoring
Mather’s TCU is staffed by a highly skilled, experienced team of healthcare providers including pharmacists, physicians, registered nurses, physical/occupational therapists and a speech therapist, registered dietician, certified social worker and recreational therapist. TCU patients have access to all the medical staff specialties, as well as the pharmacy, clinical nutritional services, respiratory therapists, diagnostic imaging, the diagnostic laboratory, etc.
Mather’s TCU is a beautifully appointed unit with hardwood flooring and an openness that makes patients feel at home. It is a healing environment that encourages mobility and independence. The self-contained unit has four private and six semi-private rooms, a bright and cheerful therapy room and a dining room/common recreational space where patients can visit with family and friends.
