The Transitional Care Unit (TCU) at John T. Mather Memorial Hospital was ranked among the nation’s best nursing homes by U.S. News & World Report’s annual Best Nursing Homes rankings.

The 16-bed TCU at Mather Hospital is a short-term healthcare facility where Mather patients receive the focused care they require before being discharged home or to another facility for those requiring extended therapies. Mather’s TCU, which fosters independence among patients, was one of the first such units in New York State.

"I congratulate our TCU staff for the high quality of care that has once again earned them this prestigious distinction," said Mather Hospital President Kenneth Roberts.

Jefferson’s Ferry, a Life Care Community in South Setauket, was also cited on U.S. News’ 5-star list. Jefferson’s Ferry has a skilled 60-bed nursing facility, residential housing and assisted living and is sponsored by Mather Health System, a 501(c) (3), not-for-profit corporation.

The Best Nursing Homes 2015 ratings highlight the top nursing homes in each city and state, out of nearly 16,000 facilities nationwide. The rankings are available at http://health.usnews.com/best-nursing-homes.

The data foundation for Best Nursing Homes 2014 came from the federal Centers for Medicare & Medicaid Services in January 2014. CMS oversees federal payments to nursing homes and assigns the star ratings described above. U.S. News awarded the "Best Nursing Home" designation to homes that earned an overall rating of five stars from CMS
